Illness Specific Return to Work / Health Conversation Forms

Customised return to work forms for specific illnesses offer a tailored approach to replace, where appropriate, the standard return to work form. The aim is to have better conversations with more support. The customised forms can be used during return to work discussions or any other relevant health discussions with employees. 

The main changes to the forms are as follows:

  • Form title changed to ‘Attendance Management Return to Work/Health Conversation Form.
  • Specific forms introduced for Cancer, Digestive, Stress/Mental Health and Musculoskeletal. The general form remains for all other illness reasons.
  • The bulleted list of checks the manager should undertake with the employee has been reordered to move the focus to support.
  • Additional links added e.g. to PAM Medicine Chest which has a wealth of health advice and support tailored to the health reason for the specific form.

Paper versions of the forms below can be downloaded. On iTrent, the specific form will download when one of the following illnesses is entered Cancer, Digestive, Stress (Mental Health) and Musculoskeletal.

Where the original or most recent reason for absence is different to the reason on the subsequent fit note, the manager should always update the reason in the iTrent payroll system and ensure that the correct Return to Work / Health Conversation Form is used.

The HR How to Guide was changed last year to emphasise the importance of communication and supporting employees with health issues to maximise attendance.

 

Return to Work Forms

AM60A Return to work form (Non Teaching) - For Managers to prepare for and to record the outcome of a return to work discussion.

 

AM60C Return to work form (Non Teaching) (Cancer related illness) - For Managers to prepare for and to record the outcome of a return to work discussion where the reason for absence was Cancer related.

 

AM60 D  Return to work form (Non Teaching) (Digestive related illness) - For Managers to prepare for and to record the outcome of a return to work discussion where the reason for absence was Digestive related.

 

AM60 M  Return to work form (Non Teaching) (MSK related illness) - For Managers to prepare for and to record the outcome of a return to work discussion where the reason for absence was MSK related.

 

AM60 S  Return to work form (Non Teaching) (Stress related illness)- For Managers to prepare for and to record the outcome of a return to work discussion where the reason for absence was Stress related
